Payment and Insurance

Payment is due in full at time of service for services rendered via cash, check, or major credit card.  My office currently participates and is considered an “In-Network” provider for the Cigna and CareFirst BlueCross/BlueShield network, including PPO and most HMO plans. This means that services are generally covered for Cigna and CareFirst BlueCross/BlueShield members, aside from any deductibles or co-pays. Please check with your insurance company for your specific benefit coverage and mental health coverage verification. You will be responsible for any service fees not covered by their insurance company or plan.    


We are considered an “Out-of-Network” provider for individuals who are not members of the Cigna or CareFirst BlueCross/BlueShield networks, however, most PPO and Open Access Insurance Plans cover therapy services provided at my office. HMO and Medicaid plans generally do not cover our services.  Clients are responsible for verifying their specific benefit coverage with their insurance company.  


Reimbursement rates for services vary between insurance companies, thus I have provided the CPT codes above. You should use these codes when you contact your insurance company to verify your benefits and reimbursement rates. The most often used CPT code at my office is “90837.” For clients who wish to pursue reimbursement, I will provide you with a monthly invoice, or SuperBill, which may be submitted to your insurance company for direct reimbursement according to your insurance company’s reimbursement fee schedule.    


There are various reasons why it may be advantageous or disadvantageous to bill through your insurance or to pursue reimbursement. At Full Circle, I strive to work from a strength-based perspective to utilize and highlight your specific strengths in treatment. Because insurance companies work from a more rigid, disease-based model of diagnosis and service delivery, this has the potential to cause the preferred treatment to conflict with what an insurance company may deem “necessary” or “appropriate” for your individual presentation or problem. Further, it is important to note that billing through an insurance company requires an active mental health diagnosis. The insurance company may also occasionally require access to a client’s most personal and sensitive information and session notes to verify or justify a certain condition or treatment. This becomes part of a client’s personal medical record and may potentially affect other aspects of the client’s life, such as employment, security clearances, or life insurance rates.  It is important to carefully consider your own personal situation and decide which course of action is best for you.

If you do not show up for your scheduled therapy appointment, and you have not notified us of your intent to cancel at least 24 hours in advance, you will be required to pay the full cost of the session as indicated above. Cancellation requests should be made via phone, email, or text message.
